What DHA does to brain cell membranes

Docosahexaenoic acid (DHA) is an omega-3 fatty acid that makes up 13 to 15 percent of the phospholipid pool in the cerebral cortex. It is concentrated in synaptic membranes and photoreceptor cells. DHA maintains membrane fluidity, receptor function and signal transmission speed. The brain can synthesize small amounts from alpha-linolenic acid, but conversion is inefficient. Dietary intake becomes the primary source.

How it works

DHA is incorporated into neuronal membrane phospholipids. It increases membrane fluidity, which affects neurotransmitter receptor density and ion channel function. Unlike citicoline, which supplies choline for acetylcholine synthesis, or phosphatidylserine, which maintains membrane structure, DHA is a structural building block. It also supports anti-inflammatory signaling through resolvin and protectin pathways.

Eicosapentaenoic acid (EPA) is the other major omega-3. EPA has stronger evidence for cardiovascular health and mood regulation. DHA has stronger evidence for brain structure and memory. Most fish oil supplements contain both.

The 1.16 g trial: memory and reaction time in young adults

Stonehouse et al. (2013)

One hundred seventy-six healthy adults aged 18 to 45 with low baseline DHA intake were randomized to 1.16 g of DHA daily or placebo for 6 months. Cognitive function was assessed with computerized tests measuring episodic memory, working memory and reaction time.

The DHA group showed significant improvements in episodic memory and working memory compared to placebo. Reaction time also improved. The benefits were most pronounced in participants with the lowest baseline DHA levels. This trial is notable because it studied young healthy adults rather than elderly populations with cognitive decline.

The 900 mg trial: learning in older adults with decline

Yurko-Mauro et al. (2010)

Four hundred eighty-five older adults with age-related cognitive decline took 900 mg of DHA daily or placebo for 24 weeks. The primary outcome was the Paired Associate Learning (PAL) test. The DHA group made fewer PAL six-type errors compared to placebo (P=0.03). Learning and memory function improved.

The negative trials: why AREDS2 and Quinn found nothing

Quinn et al. (2010)

Two hundred ninety-five participants with mild-to-moderate Alzheimer disease received 2 g of DHA daily or placebo for 18 months. The trial was published in JAMA. Cognitive decline was measured with the ADAS-Cog and CDR-SB. There was no significant difference between the DHA group and placebo at any time point. DHA did not slow cognitive decline in this population.

AREDS2 (2015)

Three thousand five hundred one older adults with age-related macular degeneration (mean age 72.7) took 350 mg DHA plus 650 mg EPA daily or placebo for 5 years. Cognitive function was assessed annually. The omega-3 group showed no significant difference in cognitive decline compared to placebo. The trial was large, long and well-controlled.

Why the mixed results

The positive trials enrolled participants with low baseline DHA or age-related cognitive decline. The negative trials enrolled participants with established Alzheimer disease or mixed populations. DHA may support memory maintenance in healthy or mildly impaired brains but cannot reverse neurodegeneration. Shahinfar et al. (2025) meta-analysis of 58 RCTs found high heterogeneity (I-squared up to 99.4 percent), meaning study design, population and dose vary so widely that pooling results is difficult.

DHA vs EPA: which one matters for memory

DHA and EPA are both omega-3 fatty acids but serve different roles. DHA is the primary structural omega-3 in the brain. EPA is more active in cardiovascular and anti-inflammatory pathways. For memory specifically, DHA has the stronger evidence. EPA may support mood and cardiovascular health, which indirectly benefits cognition, but direct memory trials focus on DHA.

Most fish oil supplements contain both DHA and EPA. Algae oil supplements contain DHA only. For memory, look for products with at least 500 mg DHA per serving. The 1.16 g dose from the Stonehouse trial is higher than most over-the-counter products provide in a single serving.

What to look for on the label

DHA content and form

Check the milligrams of DHA per serving, not just total omega-3. Some labels list 1,000 mg fish oil but only 200 mg DHA. Look for the triglyceride form or re-esterified triglyceride form. These have better absorption than ethyl ester form. Third-party testing for oxidation, heavy metals and contaminants is important because fish oil degrades easily.

Algae oil is a DHA source for people who avoid fish. It provides DHA without EPA. The evidence for algae-derived DHA in memory trials is limited but growing.

Bleeding risk
